PCNL approaches: Supine

In the beginning of the 1980s, PCNL has been developed in prone position. The more natural supine position was considered a very high risk of colonic perforation during the time when computerized tomography (CT) scan nor ultrasound was available.
Thanks to Prof. José Gabriel Valdivia Uría, the supine position for PCNL was described for the first time in 1987 and then progressively spread around the world by Spanish and Italian endourological schools.
After more than 2 decades of supine PCNL, potential limitations were resolved. The outcomes are equal, if not superior to prone PCNL thanks to superior ergonomics which eases the systematic integration between flexible ureteroscopy (URS) and PCNL itself.
